As you may be aware DCHS is piloting a process to provide a more robust governance process for questionnaires/ surveys that our staff wish to send to patients, carers and members of the public.
Service/ staff wishing to conduct a questionnaire/ survey should complete the attached pro forma and send to the Clinical Effectiveness Team (CET) at email@example.com
CET will then co-ordinate a questionnaire panel who will:
• Approve and / or edit the questions to ensure they are ethical, accurate, readable and will achieve the aims of the survey
• Approve and / or edit any associated documents to ensure they are ethical, accurate, readable and meet their requirements
• Approve and / or edit the methodology to ensure it is effective, manageable and will achieve the aims of the survey
• Make any other recommendations required
The questionnaire panel will aim to have reached a decision about the survey within two weeks of receiving a submission. Once the questionnaire panel have approved the final questionnaire & methodology the relevant divisional governance group will be notified as part of the Clinical effectiveness update paper. The divisional governance group will instruct the service wishing to conduct the survey if & how they want to receive any lessons learned.
