Lessons Learnt Panel
The current patient safety focus nationally as well as within DCHS is the shift from Safety I to Safety II. This is where we learn from all care that we deliver. The Lessons Learnt Panel are central to this but the ethos of Safety II could be expended to embrace learning from all activities that we undertake in DCHS. For learning to be achieved there must be an open and honest culture which fosters clinical curiosity of how things go right and a fair response when things go wrong. We must provide our staff a safe and supportive environment, which allows them to be transparent without fear of retribution. Staff are to be encouraged to learn collaboratively and, if challenge to current practise is required, that it is done in a kind and respectful way. This would incorporate all aspects of the Quadruple Aim as depicted in the visualization below:
The Lessons Learnt Panel will use this model to generate the items for their agenda going forwards.
