PPOC changes from 5 July - for Integrated Care Services (ICS) staff only
Following feedback from our clinicians we have implemented changes to the PPOC. Below are the sections which will change from 5 July 2021 and these changes will be implemented to all units. Please remember the prior checks and baseline assessments should be completed for all patients.
Changes – new set out as below:
- Incorporated better understanding of Prior Checks
- Diversity Monitoring implemented
- Pain assessment changes to instructions
- Analysis and Plan re instated
Reason for Referral – incorporated dependency score and guidelines as below – this is a drop down list please see guidance for clarity.
Analysis and Plan introduced – to enable clear process for clinicians to follow.
Therapy assessment now incorporated – Pre admission status Pathway 2a/2b for CSB or ward completion. What was the patient’s ability prior to admission rather than current ability
