Integrating care to improve discharge planning
Care coordinators working in GP surgeries around Derbyshire will soon be able to view real-time information about their patients while they are on wards in Derby’s hospitals.
Upcoming changes to the e-whiteboard system used by ward staff will mean the care coordinators can access relevant information about their patients, and engage in discussions with the Trust over their care and discharge planning. GPs, community matrons and district nurses will also be able to view the e-whiteboards, enabling them to access information to allow better planning for integrated care upon discharge.
This team view will be customised for the users’ needs, and will only feature information about patients who are under the care of that particular GP surgery.
It offers transparency and continuity across all care teams working in a patient’s pathway, reducing delays in transfer and should lead to safer and more efficient discharges to health and social care.
A pilot scheme will run for four weeks from 24 October 2016 at Charnwood Surgery, Hollybrook Medical Centre and White moor Medical Centre and it is hoped the change will be rolled out to all other practices in Southern Derbyshire CCG from 28 November 2016.
Full training will be provided to all users of the system.
