Important information about ICS documentation reviews from Outstanding Way

We are listening to clinicians and working hard to reduce frustration, minimise duplication and remove barriers. We understand one of the major sources of frustration is ‘documentation’, so the OW team, helped by clinicians, have committed to reviewing all TPP/S1 ICS documentation..  They started with the Initial Contact Form, which has now been replaced by the Patient Plan of Care (PPOC) and will now tackle the remaining documentation on a rolling basis of review, re-design and road-test looking at a bundle of documents that loosely belong together under care pathway headings eg falls and frailty or tissue viability. This approach will also support clinicians to use their clinical reasoning. Once the new documentation has been successfully piloted it will be deployed more widely on SystmOne.

 

The Falls and Frailty documents are ready for testing and will be piloted by named individuals within the ICS teams in Erewash, Dales, Chesterfield, Swadlincote and Derby City ONLY  from 30 October until 1 December. The new documentation has been developed by clinicians and will be tested by clinicians across a number of geographical areas to give us the best possible feedback. We are looking forward to hearing what staff say.

 

Please note: Only those staff who have been nominated should use the test documentation. They have been given guidance on what to expect and how to use the forms. Please DO NOT use unless otherwise instructed.
