Staff administering low molecular weight heparin
There have been a number of recent Datix reports where the wrong dose of LMWH (Enoxaparin, Tinzaparin) has been administered to patients.Staff are reminded to follow the ‘Six Rights’ of medicines administration, to read the dose aloud and discard any excess medication before injecting.
Staff are also reminded that: Treatment doses of LMWH may need rounding to the nearest measurable volume.
e.g. Tinzaparin 12,250 units give 0.6ml (12,000 units) of a 20,000 unit per ml syringe.
Similarly Enoxaparin 10,800 units give 0.725ml (10,875 units) of a 12,000 unit (120ml) / 0.8ml syringe.
