Funding health and social care in Derbyshire

NHS organisations and local councils have come together to develop plans for the future of health and social care. Derbyshire’s Sustainability and Transformation Plan (STP), Joined Up Care Derbyshire, brings together work that has been taking place across the county to coordinate services better to support people in staying well.

All 11 NHS and social care partner organisations working on Joined Up Care Derbyshire will look at improving how they work together, supporting people who are most at risk and making sure health and social care professionals get the best opportunities to help people lead happier, healthier lives.  Services will focus on looking after people in their home or local area, so they get the targeted care they need to get better more quickly, instead of concentrating on providing services from specific buildings.

Joining up care is also about preventing people from getting ill in the first place by helping them take good care of themselves and be able to deal with issues before they become bigger problems.

The team have produced this  infographic to help explain the challenges they are facing  when considering funding health and social care in Derbyshire.
